Carrier Profile Form Includes:

Company Name
Owner/Operator Name
MC/DOT Number
Phone Number & Email
Equipment Type (Dry Van, Reefer, Flatbed, etc.)
Preferred Lanes/Regions
Insurance Information
Dispatcher Contact Information

Step 2

Complete the Onboarding Packet

We’ll need a few important documents to get you started.

Required Documents Upload:

Signed Carrier-Dispatcher Agreement
W-9 Form
Proof of Insurance (COI)
MC Authority Letter
Driver’s License
Power Of Attorney
Notice of Agreement (NOA) regarding Factoring

You're Cleared to Roll

Once your documents are reviewed, we’ll reach out to schedule a brief onboarding call and get you moving with quality loads.

Please return the complete Dispatcher Packet, which must include the following:

✔️ Signed Dispatcher Agreement
✔️ Completed Carrier Profile
✔️ Executed Limited Power of Attorney
- Carrier is responsible for getting all legal documents notarized
✔️ Copies of the following supporting documents:
- MC Authority Letter (Certificate)
- Certificate of Insurance (COI)
- IRS Form W-9

If you are partnered with a factoring company, kindly include a copy of the factoring company’s credit application. This will allow them to perform a credit check on brokers and, if favorable, issue a Notice of Assignment (NOA).
